- Description: Provides comprehensive geriatric assessment for frail, older individuals with multiple or complex medical, psychosocial, or functional problems.Coordinates care with family/caregivers and community services to optimize functional status. Assessment service considers functional abilities, health, diet, and living situation and is aimed at maintaining self-sufficiency. The assessment team consists of physicians, public health nurses, nutritionists, and social workers. Both inpatient and outpatient programs are available.Patients must be accompanied by a caregiver if the patient is a poor informant.

- Eligibility: Anyone 75 years or older (with multiple medical, functional, psycho-social problems that exceed the ability of clinicians to care for in a standard primary care appointment). Anyone younger than 75 years if they have cognitive declination, dementia, falls, or failure to thrive.
