    Provides children, women, and families who are low-income and at high risk for adverse outcomes with health assistance. Offers case management, coordination of services, and comprehensive quality healthcare. Addresses factors which impact social determinants of health. The goals of the program are to:

    • Increase access and use of health an social services (cross cutting)
    • Improve preconceptions of healthy by decreasing risk factors for adverse life events among women
    • Reduce infant morbidity and mortality
    • Increase the proportion of adolescents and women of reproductive age who maintain a healthy weight
    • Improve the cognitive, physical, and emotional development of all children
