Offers information and counseling regarding available long-term support options Assists individuals in determining their eligibility for public long-term support programs and benefits Provides short-term case coordination to stabilize long-term support for individuals and their families in times of immediate need Helps people plan for their future long-term support needs Provides information about and referral to other programs and benefits (such as health promotion and disease prevention, transportation services, housing, and income support programs) that help people remain in the communityAging and Disability Resource Connections (ADRC) are collaborations between local Area Agency on Aging (AAA) offices and Independent Living Centers (ILC) working together with a community network of public, private, and non-profit agencies to form No Wrong Door (NWD) Systems that assist individuals in navigating the complex system of long-term services and supports (LTSS)/ community resources.ADRC of Sacramento County is a collaboration between Agency on Aging \ Area 4 (AAA4), Resources for Independent Living (RIL), and ACC Senior Services.
