- Description: SeniorLink is a prevention and early intervention program designed to provide support services to older adults (age 55+) experiencing isolation and/or early signs of depression or anxiety. - Eligibility: Older adults (55 years of age or older) who are residents of Sacramento County and are experiencing isolation and/or early signs of depression or anxiety. Eligible individuals cannot be receiving services through Sacramento County's Mental Health Plan or be enrolled in an intensive case management program.
