- Description: Helps adults struggling with mental health and/or substance abuse challenges. CORE provides clients with access to mental health and supportive services. Staff use a strengths-based approach to help clients regain control of their lives.Services offered by CORE are voluntary and include, but are not limited to: Medication Support Entitlement Support through SMART Program (GA, SSI, SSDI, etc.) Linkage to Primary Health Care Individual Therapy Support Groups Case Management Outreach and Advocacy Community Wellness Centers Referral services to programs to assist with interim and long-term housing
