- Description: Provides a wellness and recovery model program to older adults who live with a severe mental illness and are homeless or at risk of homelessness. The following services are provided at client-friendly locations: Evaluations, assessments, and individualized treatment Intensive case management with community and Personal Care Provider (PCP) referrals Mental health and substance abuse services Crisis intervention offered 24 hours, 7 days a week Medication management Nursing services Health education Family support and education Housing and transportation assistance Employment assistance and training Homeless outreach
