- Description: Provides supportive services to older adults and individuals who live with blindness, a disability, and who are limited in their ability to care for themselves and cannot live at home safely without assistance. This includes people who would be able to return to their homes from hospitals, nursing homes, or board and care homes, if they had help at home. Recipients will hire, terminate, and supervise their own provider. The recipient's social worker will approve the types of services to be provided and the amount of time to be paid based on a needs assessment. The actual provider payments are issued by the State of California directly to the provider. Some of the services that may be available include: Household and heavy cleaning Meal preparation Laundry Reasonable shopping and errands Personal care services (such as feeding, bathing, bowel and bladder care, dressing, and other services) Assistance with transportation for medical appointments and health related services Paramedical care services
