- Description: Serves older adults, adults with disabilities, and family caregivers. Provides critical services and community resources one needs to stay independent.Services include: Information, referrals, and help for older adults and family caregivers. Multipurpose Senior Services Program (MSSP) for Med-Cal eligible seniors ages 65+. Provides in-home care management from simple personal care to comprehensive care management. HICAP (Health Insurance Counseling and Advocacy Program). Provides Medicare guidance. Call HICAP or email passageshicap@csuchico.edu. List of subsidized housing for seniors. Call to have a list for Tehama County emailed or mailed. Long-term care referrals. Call or see list of facilities on website. Long-term care ombudsman. Advocates for residents of nursing homes and long term care facilities. Call or email ombuds@csuchico.edu. Fall Prevention Program. Provides home modifications such as: Ramps Shower conversions Wheelchair ramps or railings Other modifications with approval from the director Also provides safety tips and instruction on how to make home safe. Call 530-898-5923. Limited iPad distribution for older adults (60+) and adults with disabilities. This program is for those with no mobile computer devices and/or no internet service. Aims to help one access: Telehealth and tele-mental health services Companionship Suicide prevention resources. Limited supply. Call to sign up for waitlist. Program ends December 31, 2023.
