  • Description: The Housing and Disability Advocacy Program (HDAP) is a locally administered program that provides housing supports and disability benefits application assistance and advocacy to people likely eligible for disability benefits. The HDAP is intended to support participants in meeting two important program goals: securing disability benefits and stabilizing in permanent housing. Priority is given to serving people with the highest needs and vulnerabilities, which includes individuals who are experiencing chronic homelessness or with severe health conditions. Individuals, youth, and families with a disabled child may be eligible for HDAP if they are likely eligible for disability benefits and are: 1) Chronically homeless;2) Homeless; or3) At risk of homelessness; and4) Not currently receiving disability income including Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Social Security Retirement or Survivors Benefits, Veterans' Disability Compensation benefits, and the Cash Assistance Program for Immigrants. HDAP is accepting referrals from the community for people who may be eligible. Applicants will be screened for eligibility with consideration given to those most vulnerable. Please use the attached HDAP Community Referral Form, and email to HDAP staff: hdap@co.humboldt.ca.us
