  • Description: PurposeThe purpose of the Transportation Assistance Program (TAP) is to assist individuals and families who can be better served by relocating to a destination that provides a reconnection to family or friends or with employment opportunities that will lead to self-sufficiency. The program provides relocation services to individuals and families that request assistance with relocation to a new place of residence. Social Services staff verifies that a friend or family member or an agency at their destination will receive and support the individual or family. Relocation services can also be provided to an individual or family if there is verification of an offer of employment with a verified start date. DHHS can assist you in:. Referring you to services to assist with any medical conditions that may prevent you from traveling. Contacting friends or family who are willing to offer you support. Obtaining Identification

Be able to travel, if you or anyone you apply for has a medical condition or any other related issues, you will need to take care of those first before you travel.. Be able to verify within two days of applying that you have family and/or friends willing to offer support and assistance or a new job with a start date.. Have a social security number, as well as the social security number and date of birth for anyone else you are applying for.. Have less than $150 in cash, checks, bank accounts or other available funds.. Submit a completed application to DHHS. Be 18 years of age or older. Provide IdentificationIf you are eligible for transportation assistance, you may be able to receive:. Gas money if you have a valid driver's license and a registered and insured vehicle. A nonrefundable bus ticket to a pre-determined destination. A daily meal allowance for each day of travel.
