With a member who is 18-61 years AND has a disability; AND2. Is: Transitioning out of institutional/segregated settings; At serious risk of institutionalization; Homeless; OR At risk of becoming homeless.For an RHA Mainstream Voucher, the applicant will also need documentation of:1. Connection with current supportive services, including an identified case manager for a minimum of six months after being housed; and2. Completion of the local Ready to Rent Program or approved equivalent program.
