  • Description: The California Immunization Registry (CAIR) is a secure computer system that stores immunization (shot) records.How Does a Registry Help You Keeps track of all shots and TB tests (skin tests/chest x-rays), so you don't miss any or get too many Sends reminders when you or your child need shots Gives you a copy of the shot/TB record from the doctor Can show proof about shots/TB tests needed to start child care, school, or a new jobCOVID-19 Vaccine Records or Lost COVID-19 Vaccine CardIf you are looking for your COVID-19 vaccination record, please allow at least 2 to 3 weeks after your vaccine is administered to complete the online form to request it to allow the administering site time to report to CAIR. https://cairforms.cairweb.org/AuthorizationToRelease/AuthorizationToRelease Once your record is in CAIR and released, it will not arrive in an official letterhead and you will NOT receive a replacement card, but the COVID-19 vaccination information will be in the record. Once you lose your original vaccination card from the CDC, there isn't one available for us to replace it with. Additionally, the record you receive will contain ALL immunizations stored in your immunization record in CAIR.
