Care Management Services Include the following: Aging care plans and assessments Living environment evaluations and recommendations Attend physician's appointments and advocate during emergency room visits or hospitalizations Advocate with health and industry professionals to ensure quality care Oversee and manage clients living in their homes, assisted living communities, or skilled nursing facilities Act as intermediaries to help families reach mutual consensus Relieve family members of the day-to-day demands of providing care management Crisis Intervention 24/7 for active clientsDoes not provide housing.
