- Description: Provides assistance with housing placement, including offering United States Department of Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) vouchers to veterans experiencing homelessness. Also assists with the initial Veterans Affairs medical exam, healthcare eligibility, Veterans Affairs benefit application, counseling, lockers, transportation to service providers, and linkage to Veterans Affairs and non-Veterans Affairs providers. Will assist veterans experiencing homelessness with obtaining a copy of their DD214 and/or Official Military Personnel File. 24-hour information and referral also is available through the National Call Center for Homeless Veterans.

- Eligibility: - All ages; Veterans who are experiencing homelessness or who are at risk of homelessness and their families; To receive United States Department of Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) vouchers: Veterans must be eligible for Veterans Affairs (VA) healthcare, meet the Department of Housing and Urban Development's (HUD) definition of homeless or at-risk of homelessness, meet income requirements, and must need case management
