• Eligibility: The California Children's Services (CCS) program provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children under the age of 21 with a CCS-eligible medical conditions. Age 21 or under Reside in Mariposa County 3. Has a chronic, disabling, or life-threatening medical condition that is a CCS eligible medical condition Has one of the following: Family Income (AGI-Adjusted Gross Income) $40,000 or less Family income (AGI-Adjusted Gross Income) over $40,000 and out-of-pocket medical expenses are expected to be more than 20 percent of the family income (AGI-Adjusted Gross Income) A need for a diagnostic evaluation to find out if there is a health problem covered by CCS Was adopted with a known health problem that is covered by CCS A need for the Medical Therapy Program (children and youth can get Medical Therapy Program services no matter what their family income is) f. Medi-Cal, with full benefits (Full-Scope or Shared-Of-Cost)
