Individuals may need to pay a co-payment for services depending on the service and their personal situation. HealthPAC is not health insurance. • Eligibility: Are a resident of Alameda County, and Have limited income (income below 200% of
  the Federal Poverty Level), and Have no health coverage, and/or your health
  coverage does not cover the service you need, and Complete the HealthPAC application
• Requirements: Must show proof of identity, Alameda residency, and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
